Are you requesting records from Life Without Barriers:

Everyone who we support and carers, both current and former, have the right to access records held about them by Life Without Barriers. The Request for Records form captures information about the records being sought. The request can be made by individuals, family members, other stakeholders, and third parties. The release of records relies on informed consent or authorisation from the person the records are related to, as well as complying with privacy provisions and other legal requirements.

Whilst we do need sufficient information to identify records and respond to the request, if you wish to subsequently make a complaint about the way in which we have dealt with your request, you can choose to remain anonymous.
