What support coordination services does Life Without Barriers provide?

Through the NDIS, there are 3 levels of support coordination that can be included in your NDIS plan.

At Life Without Barriers, we offer Level 2 and Level 3. We can:

  • Connect you to NDIS and other government services to ensure you receive maximised support.

  • Implementation of your plan following Request for Service being received from NDIS.

  • Help you negotiate supports within your plan budgets or gather evidence to request a new NDIS plan.

  • Monitor your plan spend together and upskill you on the different types of plan management.

  • Develop plans and contingencies for more support options, especially if your needs change regularly.

  • Provide hospital to home and ongoing services. Liaising directly with hospital, NDIS Hospital Liaison officer to ensure that you have the supports in place upon you returning home.

  • Engage with mental health services in the area relevant to your home address and provide support to ensure needs are being met.

  • Secure Extended Care Units (SECUs) provide medium to long-term inpatient treatment and rehabilitation for people who have unremitting and severe symptoms of mental illness or disorder. These units are located in hospital settings.

  • Multiple Agencies Complex Needs Initiative (MACNI) in supporting clients where in the past services may have broken down (this could be due to a number of factors) and either through consultation or ongoing support significant outcomes have been achieved for the people we support.


Steps taken to complete are as follows

  • Either a face-to-face or virtual engagement meeting to ascertain the needs and discuss how your plan can be implemented.

  • Discuss supports funded and provide choice of provider unless the participant already has this support and wishes to continue with them.

  • Minimum monthly contact- dependent on individuals needs.

  • Provide you with monthly financials following receipt of plan manager monthly statement to ensure you are on track with your spending and funding will last to end of plan.

Our Support Coordinators are independent and ensure that you are the decision maker, every time. We understand the importance of choice and always provide you with a range of potential service providers.

If you opt for both Support Coordination and other services from us, rest assured, we handle these services independently to eliminate any potential conflict of interest.
