There are no right or wrong answers. Life Without Barriers will look at the answers we receive and will share with you what we learned and what action we will take.
Accessing the survey
You can do this survey in different ways. Choose the way that is best for you:
You can fill in the online version of the survey via the blue buttons below.
You can call us on 02 4033 4777 and answer the survey questions or request a paper copy of the survey. The person you talk to will be a Life Without Barriers staff member who will not share your personal information, unless you allow for us to do so.
You can complete an Easy Read version of the survey. Click here to access the Easy Read survey.
You have until the 6th June 2025 to complete the survey.
Life Without Barriers has made a promise to support your Rights.

Feedback and Complaints
You have the right to always be respected and safe when you get support from Life Without Barriers. If you are unhappy, feel hurt or unsafe, you can tell us. We will try to fix the problem.
